Abstract

Secondary students with disabilities need more than a well-written plan for life after high school. They need consistent access to instruction, communication, self-advocacy, functional skill development, and meaningful postsecondary preparation across classrooms and community settings. This white paper presents the Inclusive Transition and Communication Support Framework (ITCSF), a proposed district-oriented model for connecting those supports through shared instructional routines, educator preparation, and measurable outcomes.
